Please assist your patient in submitting a referral along with documents supporting their diagnosis (such as specialists’ consultations, imaging, bloodwork, failed treatments). If a referral is not possible, please call us for further information
After a detailed assessment by our physicians, patients will be considered for therapy with medical cannabis or cannabinoids. You will receive a consult note from our medical doctors with an outline of our treatment plan, regardless of your patient’s eligibility.
Eligible patients will be followed by our clinic with subsequent visits for continued cannabinoid therapy. We are happy to provide these visit notes upon request.
